California Department of Corrections and Rehabilitation (CDCR) has exciting opportunities for health care professionals! Due to a recent court order, CDCR has expedited the hiring process and provided additional pay incentives for the classifications listed below.
For each classification, everything you need to know and submit to CDCR is listed below:
Examination BULLETIN – This document provides details on how to apply for an examination and salary information – including pay incentives, minimum qualifications and where positions are located, along with other relevant information.
State Application (STD. Form 678) – For every examination that you are taking, please download the state application, complete it and submit it with the examination documents. EXCEPTION: RN, Physician and Surgeon (Internal Medicine/Family Practice), Physician Assistant and Nurse Practitioner examinations are taken online; therefore no state application is required.
Testing Instrument (Examination) – The column provides you with examination information. Any examination listed as “SA” (Supplemental Application) can be downloaded by selecting the link. The “SA” needs to be completed in addition to the State Application. Print the Supplemental Application examination, complete it and mail it to the address listed on the bulletin. Any Testing instrument with “EE” (Education and Experience) indicates that the examination consists of an evaluation of your education and experience as stated on your State application. You can download the State Application, complete it and mail it to the address provided on the Examination Bulletin.
Required Form – Credentials Verification Disclosure Form (CVDF) - If the CVDF is listed next to the examination that you are taking, please download, complete it and fax it along with your current Curriculum Vitae (CV), to CDCR, Credential Coordinator at Fax No. (916) 322-3208, AT THE SAME TIME you submit your State Application and Supplemental Examination Application (SA).
It’s easy – just follow the steps below:
Step 1 – Review the examination bulletin. If you determine that you meet the minimum qualifications outlined in the examination bulletin, please download the state application and complete it (link provided below).
Step 2 – Select the State Application link, download the application, print out and complete.
Step 3 – Select the appropriate link under “Testing Instrument”. If “SA” is listed, download, print it out and complete the Supplemental Application (examination) and mail it to the address listed on the Supplemental Application. If “EE” is listed, only the State Application is required—download, print it out, complete and mail it to the address listed on the Examination Bulletin.
Step 4 – If a Credential Verification Disclosure Form (CVDF) is listed next to the classification you are applying for, download the form, print it out, complete it and fax it along with your current Curriculum Vitae, to CDCR, Credential Coordinator at Fax No. (916) 322-3208, at the same time you submit your State Application and Supplemental Examination Application (SA) to the address listed on the bulletin.
